| Student Health Requirement
New students are required to complete a Medical History Form and to send it to the Dean of Students. Forms and instructions will be sent to entering students well in advance of registration. In addition, they are available from the Dean of Students. Information submitted to the Dean of Student is kept confidential and is carefully reviewed to help provide individualized health care.
Mandatory Health Insurance: Mandatory health insurance is required as a non-academic condition of enrollment for all students.
Hepatitis B Immunization: The State of California mandates that first-time enrollees who are eighteen years of age or younger provide proof of full immunity against Hepatitis B prior to their enrollment. All students who enroll at JP Catholic, and who will still be under eighteen years old by the beginning of the Fall Quarter, will receive the Hepatitis B information in the mail. The immunization consists of a series of three vaccinations. Students can receive further information from their health care provider or county health department.
Students are urged also to submit a physical examination form completed by their family physician.
